APSC 400: Technology, Engineering and Management (TEAM)

Travel Financial Authorization

Prior to travelling you must have the financial costs of this travel authorized here (link). Please provide complete details of the trip (who, where, and a breakdown of costs). NOTE: This online form is only accessible when you are directly connected to the Queen's network (it doesn't work from home).

OCASP Tips for TEAM

On the "Basic Details" page, please click on the box that says:

"I am undertaking an Activity which requires an Intermediate Review by a Principal Investigator, Activity Co-ordinator, or Departmental Reviewer, prior to final Approval by the Person-in-Authority: "

Remember that you need to click on and fill in each of the sections that says:

BASIC DETAILS chain of responsibility itinerary immunizations hazards

 

The following definitions will be used in the OCASP forms:

" Student Group " - please indicate APSC 400, TEAM # ??, Client Name - ??

"Off-Campus Acivity Leader" - this should be one of the team members travelling

" Part I -- Principal Investigator/Activity Coordinator /Departmental Reviewer" - choose "chemical Engineering TEAM projects reviewer"

PIA -"Person-In-Authority" - you should make this Jim Mclellan (OCASP.ChemEng@queensu.ca)

NOTE: The emails that are coming from OCASP at this time have a link that you are 'supposed' to be able to click on. The link looks like this:

Due to a glitch, the link does not presently work. You may need to write down, or copy and paste, the authorization code that is shown in the link. In the example above it is " 2947-1f4ba1 " If you don't do this, you wan't see the correct screens. In addition, the email address may be case sensitive, so you might wish to cut and paste that.
